[RA] Long head of biceps peritendinitis

Patient Positioning

Patient seated on a revolving stool, arm in slight internal rotation. Elbow flexed (90°), palm up.

Probe Positioning

Place the probe over the long biceps tendon between the greater and lesser tuberosities. The tendon is easier located in its short axis - then rotate the transducer (90°) in order to visualize the long axis of the tendon. Shift the probe medially and laterally (parallel to the long axis of the tendon) to examine the tendon and the synovial sheath and move the probe up from the intraarticular course of the long head of biceps and down to the myotendinous junction.

Shoulder anterior view

sp: synovial proliferation
e: effusion

Note: Peritendinitis of the long head of biceps tendon in a patient with rheumatoid arthritis with synovial proliferation and effusion (glenohumeral joint without effusion - not shown).
